Threading Problems on HLV-H

Hi All-

I'm new to threading in general but not new to the HLV-H which we've had in the shop for ten years now. We just started using it to thread various parts (it's a prototyping machine mostly so we haven't done a ton of stuff with it, a crime I know).

What I found today is that when using the 32 thread per inch setup the lead screw stopped turning mid thread! then it caught again and obviously ruined my part. This behavior did not happen the first few passes but did happen toward the end. Does anyone know why this might happen? Is it a simple procedural issue that I am doing incorrectly or do I have more serious problems?

Some relevant information is material (1144), speed (300RPM), tool cutter (Aloris HSS threading tool), thread size (10-32), compound angle (29 off center so 61 on the compound base). I'm not sure if that information is relevant to this problem or not. I was following the basic standards for threading and had success on this same part for all passes prior to the last pass when the feed stalled and ruined the thread.

This is the second threaded part I've made. Yesterday I successfully cut a 7/16-14 thread into 4140.
