Hardinge HLV Style Tailstock Problems

Long time lurker but first time poster.

Have a Victor 618 EVS lathe that from what I understand is a clone of a Hardinge HLV. Hope it's appropriate to post here in the Hardinge section.

Had some problems while trying to remove a morse taper from the tailstock this morning. I heavy handed the screw handle to release the taper and inadvertently caused the tailstock to jam. The screw handle become overly tight and hard to move.

Disassembled the tailstock hoping to see something obvious but never found it. Cleared the morse taper and went to reassemble the tailstock. Could not rethread the feed screw on to the spindle, began to force it thinking the first few brass threads of the "feed screw nut" (brass piece in the photo below) were out of alignment. Sadly that didn't work out and I badly stripped the threads on the OD of the "feed screw nut".

A few questions.

1) Is there a common failure point on these tailstocks that would jam up the screw handle and spindle?

2) Is there an order of operations for reassembly of the tailstock that needs to be followed? Had a heck of a time hammering the spindle in and out of the tailstock even with everything disassembled.

Marching towards a big parts deadline next week and couldn't have happened at a worst time.
